Complete the TSSAA physical form (dated on or after 4/15 if possible). A copy is attached below.
The "Medically eligible" page of the physical and birth certificate are the only two documents that are required to be uploaded in Final Forms.
You will not be allowed to participate in tryouts unless you are enrolled and complete (green) in Final Forms at the time of tryouts.
