Important Dates for 26-27 Volleyball Team

May 4th 3:30-4:30 Pre tryout meeting rules and guidelines

May 28th 3:00pm-5:30pm Only current 6th and 7th Siegel Middle students

May 29th 10:30am-12:30pm All other students will need to tryout on the 29th and any makeups

June 2nd TBD Parent meeting

Players MUST have their paperwork submitted to Final Forms in order to be able to participate in tryouts:

2. Concussion Statement

3. Sudden Cardiac Arrest Agreement

4. Impact Test Consent

5. Birth Certificate

6. Personal Affidavit of Responsibility

7. Transportation Agreement

8. PPE Health History Form

9. PPE Physical Examination Form

10. PPE Medical Eligibility Form
