Tryouts for the 2025-26 school season basketball team will be July 25th from 3:00-5:00pm. Doors will open at 2:30 for sign ins!

3. Sudden Cardiac Arrest Agreement
4. Impact Test Consent
5. Birth Certificate
6. Personal Affidavit of Responsibility
7. Transportation Agreement
8. PPE Health History Form
9. PPE Physical Examination Form
10. PPE Medical Eligibility Form
